Do you only get 5 stars on heists? after finishing story mode I went on a rampage trying to get 5 stars, found a good vantage point on a mountain top, got myself some rockets, nades etc and took out everything that came close for ages, only got to 4 stars.
Stroy mode as it turns out is pretty dull once you finish the story
You can get 5 stars rather easily in the online mode, I honestly haven't done much of the story mode (only enough that I could finally start online), and play online with friends here and there. You can easily get 3 stars from robbing a nearby store and shooting the clerk, then just start shooting at police and killing them more to increase your star levels.
Please keep in mind however, that police in GTA V are a massive pain in the ass to escape from.
You probably only get 4 stars from the mountain top because you're not actively harassing citizens or really in the middle of everything such as in the city.
